1. Added unauthorized extra insurance without our consent.
2. Stated policy was to return the car with an empty tank, but the car was handed over with a full tank and we were charged $105 for the fuel.
3. Agreed fee for returning the car to a different location was $600, but we were actually charged $1,000.

The local Avis branch asked if I wanted the highway toll pass, and I said yes, but they didn't explain the costs. In the end, after 21 days of rental, they charged over $300 extra on my credit card. They also upgraded the insurance without my consent.
